Державне підприємство «Державний експертний центр Міністерства охорони здоров’я України», іменований далі “Виконавець”, що є платником податку на прибуток на загальних підставах, в особі директора Департаменту економіки та фінансів Лісніченко А. М., що діє на підставі довіреності від 30.12.2011р., з одного боку, та_________________________________________________

___________________________________________________, що діє на підставі___________________________________, з іншого боку (далі - Сторони), уклали дану Додаткову угоду про наступне:
1. Доповнити Договір №______/2012/А від “___”___________2012 р. розділом 8 у наступній редакції:
« 8. ЗАЛИШКИ ЗРАЗКІВ ТА РЕАКТИВІВ
8.1.Після завершення надання послуг Виконавець зобов’язується передати, а Замовник – отримати за актом приймання-передачі залишки наданих останнім для проведення лабораторного аналізу:
8.1.1. Стандартних зразків, специфічних реактивів, матеріалів – в день отримання висновків лабораторного дослідження;
8.1.2. Зразків лікарських засобів — через 6 місяців від дати завершення надання послуг відповідно до п. 3.2.1. Договору.»
У зв`язку з чим розділи Договору 8, 9, 10 та 11 вважати відповідно розділами 9, 10, 11 та 12 даного Договору.
2. Всі умови Договору, не зачеплені Додатковою угодою, залишаються незмінними та Сторони підтверджують по ним свої зобов‘язання.
3. Додаткова угода № ____ від 01 жовтня 2012 р. є невід`ємною частиною Договору №______/2012/А від “____”___________2012р. на проведення аналізу лікарських засобів.
SUPPLEMENTARY AGREEMENT __
CONTRACT №____/2012/А

for performing analysis of medicinal products

Public Enterprise “The State Expert Center of the Ministry of Health of Ukraine” liable to income tax without exemption, hereinafter referred to as "Performer" and represented by A. M. Lisnichenko, Director, Economics and Finance Department acting according to the Power of Attorney of 30.12.2011, on the one part, and ___________________________________________________

liable to income tax, hereinafter referred to as "Customer", represented by _______________________________________

___________________________________________________, acting according to__________________________________, on the other part (hereinafter – Parties), have concluded the present Supplementary Agreement as follows:
1. To add part 8 to Contract №______/2012/А of “___”____________2012 to read as follows:
“8. LEFTOVER OF SAMPLES AND REAGENTS
8.1. After completion of rendering services the Performer shall undertake to transfer and the Customer – to take (using Transfer and Acceptance Statement) the leftover submitted by the latter for the laboratory analysis:
8.1.1. Of standard samples, specific reagents, materials – at the day of getting the results of the laboratory test;
8.1.2. Of the samples of medicinal products — within 6 months after the date of completion of rendering services according to item 3.2.1. of the Contract.»
Consequently, parts 8, 9, 10, and 11 shall be numbered as parts 9, 10, 11, and 12 of the present Contract, accordingly.
2. All conditions not covered by the Supplementary Agreement shall remain unchanged, and the Parties shall recognize their obligations concerning them.
3. Supplementary Agreement №____ of «01» October 2012 is an integral part of the Contract №______/2012/А of «___»__________2012 for performing analysis of medicinal products.
