Public Enterprise “The State Expert Center of the Ministry of Health of Ukraine” liable to income tax without exemption, hereinafter referred to as "Performer" and represented by A. M.Lisnichenko, Director, Finance and Economics Board acting according to the Power of Attorney of 04.09.2013, on the one part, and ___________________________________________

______________________________________________, acting according to_____________________________, on the other part (hereinafter – Parties), have concluded the present Supplementary Agreement as follows:
1. Parties have agreed to extend the term of the Contract №______/2012/З of «____»_____________ 2012 for the period of one year, namely till «____» __________ 201_р.
2. Item 10 of the Contract shall be read as follows:

3. Всі умови Договору, не зачеплені Додатковою угодою, залишаються незмінними та Сторони підтверджують по ним свої зобов‘язання. 4. Додаткова угода №___ від “___”_________ 2013р. є невід`ємною частиною Договору №______/2012/З від «___»__________2012р. на проведення експертизи матеріалів на лікарські засоби. | 3. All conditions not covered by the Supplementary Agreement shall remain unchanged, and the Parties shall recognize their obligations concerning them. 4. Supplementary Agreement №___ of «___» _______2013 is an integral part of the Contract №______/2012/З of «___»__________2012 for expert evaluation of materials related to medicinal products. |
