Метрологічна,4, в особі Першого заступника генерального директора ДП «Укрметртестстандарт» - директора Українського науково-технічного інституту випробувань та сертифікації електрообладнання УкрТЕСТ ДП «Укрметртестстандарт» Римера Віктора Давидовича, цим надає	The State Enterprise All-Ukrainian State Research and Production Center of Standardization, Metrology, Certification and Protection of consumer’s rights SE «Ukrmetrteststandart» (Conformity assessment body - hereinafter CAB), registered to the address: Metrologichna 4, 03680 Kiev, Ukraine, represented by First deputy of general director SE «Ukrmetrteststandart» - Director of Ukrainian scientific and technical Testing and Certification Institute of Electrical Equipments UkrTEST SE «Ukrmetrteststandart» Victor Rymer, to grant the right for
___________________________________________________________________________________________, (назва замовника / name of the customer) зареєстрованому за адресою / registered to the address _______________________________________________________________________________________ названому надалі "ліцензіатом" якого представляє / hereinafter referred to as "Licensee", represented by _________________________________________________________________________________________ (посада та ПІБ представника замовника / position, full name of the customer’s representative)
право нанесення ідентифікаційного номеру призначеного Органу (001) поряд із Національним знаком відповідності (рис. 1) на продукцію торгівельних марок: __________________________, на яку оформлені сертифікати відповідності ДП «Укрметртестстандарт»(згідно з додатками до цієї ліцензійної угоди), протягом дії цієї ліцензійної угоди, при виконанні наступних умов: 1. ЛІЦЕНЗІАТ 1.1. Зобов'язується забезпечувати відповідність продукції, зазначеної в сертифікатах, усім вимогам технічного регламенту та нормативних документів на продукцію, що наведена в сертифікатах, а також зразкам, які пройшли випробування з метою оцінки відповідності. 1.2. Зобов'язується забезпечувати безперешкодний доступ представникам, уповноваженим ООВ, до усіх ділянок виробництва продукції, що відповідає Технічним регламентам (якщо це передбачено модулем (схемою) оцінки відповідності). 1.3. Зобов'язується попередньо сповіщати ООВ про всі майбутні модернізації (модифікації) продукції, змінах у її конструкції (складі), технології виготовлення, методах випробувань і контролю, правилах прийому та змінах системи управління якістю, які можуть вплинути на сертифіковані показники. 1.4. Зобов'язується вести облік всіх рекламацій (претензій) на сертифіковану продукцію, і негайно повідомляти про них ООВ. 1.5.Зобов'язується оплачувати у встановленому порядку витрати, пов’язані з проведенням робіт з технічного нагляду відповідно до програми технічного нагляду, що є додатком[1] до цієї угоди.	the right of application of identification number of the appointed Body (001) next to the national Mark of conformity (see figure 1) for the products of trade marks: _________________, with the certificates of conformity issued by SE “Ukrmetrteststandart” (according to the annexes to the present license agreement), within the term of validity of this license agreement, and provided that following conditions are met: 1. LICENSEE 1.1. Undertakes to ensure conformity of the products, stated in the certificates with all requirements of Technical regulation and normative documents for these products as well as conformity with samples having passed through the tests with the purpose of conformity assessment. 1.2. Undertakes to provide an unimpeded access to the representatives authorized by CАB, to all sites of manufacture of products, which satisfies the Technical regulations (when this is stipulated by the conformity assessment module (scheme). 1.3. Undertakes to inform preliminary CАB on all planned modernizations (updating) production, changes in its design (structure), manufacturing techniques, test methods and the control, acceptance procedures, changes of quality manual systems, which could influence the certified characteristics. 1.4. Undertakes to maintain records of all claims for certified products and inform CАB on them immediately. 1.5. Undertakes to pay when due hereunder the expenses which are connected to work on technical supervision according to the program of technical supervision. The program of technical supervision is in the annex[2] to this license agreement.

1.6. Має право використати інформацію про сертифіковану продукцію, з метою реклами. 1.7. Має право подавати апеляцію до ООВ по всіх розбіжностях, пов'язаним з оцінкою відповідності продукції. 2. ОРГАН З ОЦІНКИ ВІДПОВІДНОСТІ 2.1. Зобов'язується виконувати технічний нагляд за виробництвом сертифікованої продукції. 2.2. Зобов'язується вчасно, не пізніше, ніж за шість місяців, інформувати ліцензіата листом (або іншим аналогічним способом) про намічені зміни в діючих нормативних документах, якими встановлені обов'язкові вимоги. 3. ІНШІ УМОВИ 3.1. Дана угода вступає в дію з моменту її підписання і залишається в силі доти, поки її не буде анульовано по об'єктивних причинах або розірвано однієї зі сторін за умови повідомлення про це інший сторони у встановлений термін. 3.2. У випадку виникнення питання про анулювання ліцензії, установлюються такі строки залежно від причин: Ситуація, що вимагає попередження про скасування ліцензії Строки завчасного попередження про скасування ліцензії Бажання виробника припинити дію ліцензійної угоди 30 днів Органом з оцінки відповідності виявлена небезпека продукції для споживачів Не встановлюється Забезпечення відповідності продукції новим вимогам нормативних документів За домовленістю Невиконання фінансових зобов'язань за договором з виконавцем робіт 30 днів Повідомлення про скасування ліцензії направляється іншій стороні листом (або іншим аналогічним способом) із вказівкою причин скасування та строку припинення дії ліцензії. 3.3. У випадках, якщо ліцензіат сповіщає ООВ про власну неготовність прийняти нові вимоги у встановлений термін; якщо порушує строки або у випадку одержання негативних результатів випробувань продукції на відповідність зміненим вимогам, ліцензія вважається анульованою з моменту введення в дію нових вимог. 3.4. Юридична відповідальність за відповідність сертифікованої продукції наведеним у сертифікаті вимогам покладається на ліцензіата	1.6. Has the right to use the information on certified products with the purpose of advertising. 1.7. Has the right to submit the appeal of CАB on all divergences connected of conformity assessment of production. 2. CONFORMITY ASSESSMENT BODY 2.1. Undertakes to carry out technical supervision on manufacture of certified products. 2.2. Undertakes in due time not later than for six months, to inform the licensee the letter (or other similar way) on the changes which are were outlined in working normative documents by which obligatory requirements are established. 3. OTHER CONDITIONS 3.1. The present agreement comes into action from the date of signing this agreement and remains in force until it will be abolished for the argued reasons or is terminated by one of the sides under condition of the notification about it of other side in target dates. 3.2. In case of occurrence of a question on a cancellation of the license such terms are established depending on the reasons: Situation which demands the prevention of a cancellation of the license Terms of the preliminary prevention of a cancellation of the license Desire of the manufacturer to terminate the license agreement 30 days The conformity assessment body reveals danger of production to consumers not established Maintenance of conformity of production to new requirements of normative documents Under the arrangement Default of financial obligations under the contract with the executor of works 30 days The message on a cancellation of the license goes to the second side the letter (or other similar way) with the indication of the reasons of a cancellation and term of cancellation of the license. 3.3. In cases if the licensee informs CAB on own unavailability to accept new requirements in target dates; if breaks terms or in case of reception of negative results of tests of production on conformity to the changed requirements, the license is considered cancelled from the moment of introduction in action of new requirements. 3.4. The Licensee should carry out the legal responsibility for conformity of certified products to the requirements indicated in the certificate.
