Диалог №1
Shona: Good morning, Mrs Chad. My name’s Shona. I‘ll be admitting you to the ward today. Would you like to come into the Patient Admission Office so I can get some paperwork done?
Mrs. Cnad: Good morning, Shona. Yes, thanks, I could do with a sit down.
Shona: Here you are. You take this chair here. You can put your stick on the edge of the chair if you like.
Mrs. Chad: Oh. Thank you, dear.
Shona: How are you today?
Mrs. Chad: Not too bad, thank you. I haven’t been waiting for too long at all.
Shona: That’s good. Now, I’m going to be taking down some details before you’re admitted to the Cardiac Unit today. I’d like to ask you a few questions, if it’s all right with you?
Mrs. Cnad: Yes, of course. That’s fine.
Shona: All right, well now, let me just get the admission form.

Shona: Right, let’s get started. Would you mind if I check out some details first?
Mrs. Chad: No not at all. What would you like to know?
Shona: I’d just like to check your name and date of birth and see if your identity bracelet is correct. Can you tell me your full name please?
Mrs. Chad: Yes it’s Doreen Mary Chad and my date of birth is the fifth of June nineteen twenti-three. Quite a while ago, isn’t it?
Shona: Not so long ago. Time goes very fast when you’re busy, doesn’t it? Right now, let’s see. Doreen Mary Chad. C-H-A-D. That’s correct, isn’t it?
Mrs. Chad: Yes that’s right. Chad with “A” “D”.
Shona: And your date of birth is the fifth of June nineteen twenty-three.
Mrs. Chad: Yes.
Shona: All right. Can you tell me why you’re here today?
Mrs. Chad: Well, um, I’ve got high blood pressure, and I’m here for some tests. My doctor asked me to come here to see what’s going on.
Shona: OK. Now I’d like to ask you about your past medical history. Have you had any serious illnesses in the past?
Mrs. Chad: Yes, I had a mild heart attack last year it was quite frightening.
Shona: Yes, I’m sure it was. Now, what about past surgical history? Have you ever had any operations?
Mrs. Chad: No, I’m very lucky. I never have.
Shona: That is lucky. Now are you taking any medications at the moment?
Mrs. Chad: Yes, my doctor put me on some blood pressure tablets after my heart attack.
Shona: Do you know what they're called?
Mrs. Chad: I don’t know, but I’ve got them here with me. I was told to bring them.
Shona: Mm. That’s good. Do you think you can show them to me, please?
Mrs. Chad: Yes, I can. I’ve got them somewhere in my bag. Here they are.
I take them in the morning with breakfast.
Shona: Right, that’s fine. You’re taking metaprolol to lower your blood pressure. I’ll just write down the name of the medication on the admission form. Metaprolol. Do you have any allergies to any medications?
Mrs. Chad: Not that I know of.
Shona: Um. What about food allergies? Any food which doesn’t agree with you?
Mrs. Chad: No, no, nothing like that.
Shona: Good, (smiles) Are you allergic to sticking plaster or iodine?
Mrs. Chad: No, I’ve never had any problems before.
Shona: All right. Can you tell me the name of your next of kin?
Mrs. Chad: It’s my son, Jeremy. Jeremy Chad.
Shona: Thanks. That’s all for me. I’ll leave you here for a minute while I get the admitting doctor to come and see you. Are you comfortable?
Mrs. Chad: Yes, thanks. I’m quite all right here.

Doctor: Have you ever had scarlet fever, measles, whooping cough or chicken-pox?
Patient: Yes, I’ve had all the childhood diseases.
Doctor: And what about adult illnesses? Have you ever had jaundice, heart or kidney trouble?
Patient: No, I haven’t.
Doctor: Have you ever had any operations?
Patient: Yes, I have. I’ve had my appendix out.
Doctor: Have you ever broken any bones?
Patient: Yes, I have. I broke my leg in childhood.
Doctor: Are your parents alive?
Patient: Yes, they are quite well.
Doctor: Do you have any brothers or sisters?
Patient: Yes, I do. I have a sister and a brother.
Doctor: Are they in good health?
Patient: My brother is in good health, but my sister has diabetes.
Doctor: Are you married? Do you have any children?
Patient: Oh, yes. I’m married. I have a son and a daughter and they seem to be all right.

Marina: I hope not, but as a matter of fact I have a headache.
Tanya: Did you take your temperature?
Marina: Yes, I did. I found out that I had a slight temperature.
Tanya: Let me have a look at your tongue. … It is coated. Besides you have a sore throat. And now let me feel your pulse. … Your pulse is rapid and irregular.
Marina: I see. That’s why I am feeling unwell.
Tanya: I think you have caught a bad flu. You’d better see a doctor.

Bill: I’m fine. And you?
Clark: I’m OK, but my sister is not feeling well.
Bill: What’s wrong with her?
Clark: She has a cold and a slight fever.
Bill: That’s too bad. I hope she’s feeling better soon.

Doctor: What’s wrong with you?
Patient: I’ve had pain here — just below my heart.
Doctor: Do you have these pains all the time?
Patient: No, I don’t. They usually come after meals.
Doctor: After meals? Do you eat much?
Patient: Well? I am very fond of food.
Doctor: That’s why you have these pains. You’d better eat not so much. What’s your weight?
Patient: About 85 kilograms.
Doctor: Try to get your weight down. Eat not so much bread. No sugar. No butter. No cake.
Patient: I’ll try, but it’s not so easy.
