10th Annual Family Forum

2016 Exhibitor Reservation Form

Value Behavioral Health of Pennsylvania (VBH-PA) is pleased to announce the 10th Annual Family Forum, Broadening Our Possibilities – Hope for Tomorrow: A Forum for Families with Children in HealthChoices. This year’s forum will be held at Ramada Inn Greensburg on Friday, October 7, 2016, from 9:00 a. m. to 3:00 p. m. Registration begins at 8:15 a. m. Forum start time will be 9:00 a. m.

Two complimentary registrations are provided for exhibitors. Exhibit space is free and available on a first come, first served basis. Set-up for exhibits will begin at 7:00 a. m. We are asking vendors to exhibit from 8:15 a. m. to 1:00 p. m. (continental breakfast included). Exhibitors are invited to attend the keynote speaker, exceptional parent/caregiver award ceremony, breakout sessions and lunch. Please be sure to indicate below if you would like a lunch reservation.

All exhibitors will be contacted when your reservation is received. If you are not contacted within two weeks of submitting your exhibit registration form, please contact Karan Steele.

The deadline for registration is Friday, September 30, 2016, and space is limited.
