Sponsorship of students undertaking post graduate distance learning course for gastroenterology

SPONSORSHIP OF STUDENTS UNDERTAKING POST GRADUATE DISTANCE LEARNING COURSE FOR GASTROENTEROLOGY

CONTRACT

It is hereby agreed that _________________________, who is commencing an 18 month part-

time distance learning course in Gastroenterology will receive sponsorship up to a maximum of

€4,341 in ’16 and €4,251 in ’17 in respect of fees from the HSCPs Unit of HSE

Payment will be made by the HSE on receipt of signed contract and invoice from IICMS.

In accepting these sponsorship arrangements, ______________________ voluntarily accepts the following conditions:

(a)  Sponsorship is reliant on successful completion by ____________________of the course

(b)  If ___________________ does not complete the Course or absents from the Course (except in exceptional circumstances, i. e. sick leave), ___________________ will repay to the HSE such monies as is deemed appropriate.

(c)  On successful completion of the Course __________________undertakes and agrees to remain in the employment of the HSE, based on the period of sponsorship, e. g. 2 year sponsorship, two year employment.

(d)  Failure to adhere to condition (c) will result in __________________ being liable to the HSE for repayment of the sponsorship paid during the course.
