Herewith I authorize International Agency of Congress Management "MAKO" to pay by the credit card for the Social program services (Conference banquet on 9 of June, 2016) within the 21st International Conference on Phosphorus Chemistry. The card information is to be put into this form to affect the payment. I agree that the payment will be charged in Rubles according to the Central Bank rate.

This form is not valid if not accompanied by photocopies of the credit card from both sides. Данная форма действительна только при наличии ксерокопий карточки с двух сторон. In case of cancellation of the application by our party this amount is not refund. В случае аннуляции заявки с моей стороны данная сумма не возвращается. The process can not be started before all payment documents are provided: authorized letter and photocopies of the credit card both sides.
