В настоящее время не существует эффективной схемы лечения СРК. При назначении лечения пациентам с СРК большое значение придаётся детальному изучению истории болезни и дифференциации групп жалоб (абдоминальных, общих, психологических и т. д.) и это в значительной степени определят тактику терапии. Важными составляющими в лечении данной патологии являются нормализация образа жизни, режима и характера питания, психотерапия. Для лечения СРК используются различные группы препаратов: спазмолитики, слабительные, противодиарейные препараты, про - и пребиотики, антидепрессанты.

Схемы лечения СРК не стандартизированы и нет единого алгоритма лечения разных форм СРК. Считается, что это обусловлено проблемой поиска и объективной оценки эффективности того или иного препарата и связано с недостаточной изученностью, сложностью патофизиологии СРК и многообразием клинических проявлений.

Но, тем не менее, некоторые препараты является перспективными в отношении использования в комплексной терапии СРК.

Среди препаратов для лечения СРК с преобладанием диареи как сильное вяжущее средство издавна использовались соли висмута. В основе эффектов препаратов висмута лежит их способность образовывать на поверхности слизистой оболочки защитные пленки, особенно с поврежденными денатурированными белками. Наличие у трикалия дицитрата висмута обволакивающих и вяжущих свойств, его эффективность в комплексной терапии больных с воспалительными заболеваниями кишечника и СРК была продемонстрирована в ряде клинических исследований [39].

К другой группе препаратов, используемых для лечения СРК можно отнести сорбенты – это вещества, обладающие высокой сорбционной емкостью, не разрушающиеся в ЖКТ и способные связывать экзо - и эндогенные вещества (микроорганизмы и их токсины, яды, излишек продуктов обмена и другие вредные для организма субстанции) путем ад - и абсорбции, ионообмена или комплексообразования. История применения энтеросорбентов уходит в глубокую древность: еще врачеватели Древнего Египта, Индии, Греции использовали древесный уголь, глину, растертые туфы, пережженный рог для лечения отравлений, дизентерии, желтух и других заболеваний. Березовый или костный уголь с этой целью использовали лекари Древней Руси. Как полагают, Авиценна впервые предложил методы энтеросорбции с профилактической целью: в своем Каноне врачебной науки, говоря об искусстве сохранения здоровья, он из семи постулатов этого искусства на третье место ставил методы очистки организма от излишков. В XVIII веке были описаны сорбционные свойства углей, а в России преемник (1757-1804) в 1785 г., изучая химические свойства древесного угля, обосновал применение метода энтеросорбции [35].

Термин «энтеросорбция» был предложен и соавт. в 1983 г. [37, 38] для обозначения нового метода сорбционной терапии, состоявшего в ежедневном пероральном приеме значительных (20-50 г) доз высокоактивных синтетических углей сферической грануляции, полученных путем пиролитической обработки различных полимерных смол.

Авторами на примере данного сорбента впервые были обобщены основные механизмы действия данного вида терапии: поглощение токсичных веществ, попадающих в желудочно-кишечный тракт перорально; поглощение токсинов, диффундирующих в просвет кишечника из крови; связывание токсических веществ, выделяющихся вместе с пищеварительными соками; поглощение токсических метаболитов, образующихся непосредственно в пищеварительном тракте; сорбционная модификация диеты; фиксация и перенос на поверхности сорбентов физиологически активных веществ (ферменты, желчные кислоты и т. д.); изменение объема неперевариваемого остатка и исходных свойств кишечного содержимого по типу, схожему с присутствием в нем пищевых волокон. Важным считается, что авторы определили классическое свойство энтеросорбентов не проникать через слизистую ЖКТ, т. е. не иметь системной фармакокинетики [38].

Среди рассматриваемого класса лекарственных препаратов особо следует выделить кремнийсодержащие энтеросорбенты, самым распространенным их которых является препарат Энтеросгель®. Энтеросгель® представляет собой синтезированный спиртовым или водным способом гель гидроокиси метилкремниевой кислоты, органофильность которого связана с наличием на поверхности раздела фаз метильных групп, а пористость (150-300 м2/г) формируется за счет пространств между микроглобулами материала, имеющими размеры порядка 50 нм и заполненными водой [28, 36]. Энтеросгель® характеризуется низкой сорбционной активностью по веществам малой и большой молекулярной массы, а по средней массе обладает выраженной селективной емкостью, но, в то же время, сорбция таких близких по молекулярной массе красителей как бромфеноловый синий (670 Да) и конго красный (698 Да) может различаться в 2,5 раза, демонстрируя наличие у Энтеросгеля® известной избирательности поглощения.

Учитывая, что Энтеросгель является высокоэффективным энтеросорбентом, нами было проведено изучение характера его эффективности и безопасности при использовании в лечении больных с СРК. В качестве препарата сравнения в данном исследовании был использован трикалия дицитрат висмута. Основанием для выбора послужило то, что данное средство может быть использовано у пациентов с СРК в течение периода лечения от 7 до 21 дня, в то время как другие группы препаратов у пациентов с Д-СРК (спазмолитические средства, антибактериальные препараты, противодиарейные алюмосиликаты) используются симптоматически или в режиме «по требованию» и продолжительностью не более 3-7 последовательных дней.

The purpose of the study

The aim of this study is to determine the efficacy and safety of using the drug Enterosgel ®, indicated for the treatment of patients with irritable bowel syndrome with predominant diarrhea.

Research Objectives

1. To assess the dynamics of clinical symptoms, laboratory parameters, data of the Electrogastrogram, psycho-emotional indicators and quality of life in patients with IBS, during treatment with Enterosgel ®.

2. To study the tolerability and safety of the drug under Enterosgel ® constant taking for three weeks.

3. Perform a comparative evaluation of the clinical and laboratory and instrumental parameters in the examined patients in the application of the test drug and the reference drug.

Materials and methods

Single-center, open, prospective, controlled study of the efficacy and safety of using the drug in the gastroenterology clinic Enterosgel® was performed in the Northwestern State Medical University. II Mechnikov.

The present study is an open (during treatment doctors, researchers and patients involved in the study knew which treatment the patient receives), prospective (patients recruited in accordance with the criteria set out in the study protocol, were observed for some time, patient groups were formed before results fixation), post-marketing (this study relates to phase IV clinical trials for the classification ICH GCP).

Criteria for inclusion of patients in the study: The study will include patients with a verified diagnosis of irritable bowel syndrome with predominant diarrhea. To participate in the study, patients must also meet the following inclusion criteria:

1. Patients aged from 18 to 60 years inclusive;

2. Patients with a diagnosis of IBS with diarrhea, according to the criteria of the Rome III;

3. Patients with the intensity of pain / discomfort in the stomach of not less than 30 mm on VAS;

4. A signed informed consent form.

Exclusion criteria were:

1. Inability or unwillingness to give informed consent to participate in the study or to fulfill the requirements of the study;

2. Clinically significant abnormal laboratory parameters, indicating the presence of an unknown disease or require additional clinical examination (by the investigator

4. Antibiotics, pre - and probiotics;

5. Availability of a stool culture pathogens (Shigella spp., Salmonella spp., Yersinia spp.);

7. Gastric ulcer and duodenal ulcer;

8. Inflammatory bowel disease (ulcerative colitis, Crohn's disease);

9. Neoplasms of the gastrointestinal tract; 10. Availability of of clinically significant allergic reactions in history;

    Availability of of severe clinically significant neurological, cardiovascular, gastrointestinal, hepatic, renal, immune and other diseases in history; 1. Mental illness, including previously deferred, which, according to the researcher, doing unacceptable patient participation in the study; 2. Drug abuse in the year prior to study entry; 3. Change in smoking habits during the month prior to the study; 4. Severe convulsive disorders in history; 5. Pregnancy; lactation; 6. Increased sensitivity to the drug; 7. Patients who are prone to failure from of _ implementation of the requirements of the doctor; 8. Patients participating in other clinical trials. Criteria for exclusion of patients from the study: 1. Waiver of further use of the drug; 2. Deterioration of objective indicators of the patient; 3. The emergence of serious adverse events; 4. The appearance of side effects; 5. Changing the diagnosis. The study included 30 patients with IBS with diarrhea. Age of the patients was 38,0 ± 10,2 years. Distribution by gender - 16 women and 14 men. Patients were divided into 2 groups: - The main group (n = 15), treatment with Enterosgel ® inside of 1 dose of 22.5 g of a paste with the active substance polymethylsiloxane polyhydrate (nonlinear polycondensation product of 1,1,3,3-tetrahydroxy-1,3-dimetildisiloksana polyhydrate) 3 times day 1 hour after meals for 21 days.

    The control group (n = 15): treatment tripotassium dicitratobismuthate. The drug was administered 1 tablet 3 times a day 30 minutes before meals _ 1 tablet at night for 21 days. All 30 patients successfully completed the study and performed the completion of of visit. There was no exclusion of patients from the observation during the study. Enterosgel ® drug was used for three consecutive weeks. Dosage and administration: Enterosgel ® orally 1 dose 3 times a day 1 hour after meals for 21 days. Throughout the study, conducted the next visit of the patient in the clinic: a screening visit, baseline visit (Visit 1) to include the patient in the study and started taking the drug, the visit of 2 three weeks after you start taking medication, visit 3, two weeks after the end of dosing.     Visit 2 (final examination) was conducted three weeks after the first visit. Evaluated adverse events, conducted standardized survey of the patient, recorded vital signs. Performed psychological testing, assessment of quality of life, pain assessment by visual analog scale.
