it is reasonable - разумно, рационально. could have been – здесь мы снова видим модальный глагол в сослагательном наклонении, т. е. автор выдвигает свою гипотезу осторожно: «разумно предположить, что аритмия могла быть результатом….», единственное слово, нарушающее эту «осторожность» - in fact (на самом деле), т. е. автор показывает настоящую, с его точки зрения, причину.

However, our present knowledge about the electrophysiological characteristics of cardiac tissues indicates that such a random distribution of refractory periods is not possible. Clearly, the strong electrical connections that exist between neighboring cardiac cells tend to greatly diminish any differences that might exist in the duration of their action potentials were such cells not connected to each other [22].

However, our present knowledge indicates that.. в сочетании с  is not possible однозначно показывает позицию автора, что современный научный взгляд считает что-то невозможным. И следующее предложение он начинает с вводного clearly (without doubt, obviously; несомненно, очевидно), не оставляя места для сомнений в его правоте.

В следующей фразе мы видим гипотетичность высказывания:

So it seems as though, if any dispersion of refractoriness exists in the myocardial mass, it probably occurs as macroscopic spatial gradients of refractoriness rather than microscopic randomly distributed temporal dispersion of refractoriness [23].

it seems, probably, as though, и rather than – «скорее, чем» показывают сомнения  автора.

Критикуя общепризнанную теорию, автор пишет:

Yet another important question that remains unanswered is what role, if any, do the three-dimensional structure and the complex geometry of the atrial myocardium play in the formation and maintenance of multiple wavelet propagation during fibrillation?

- на еще один важный вопрос, по мнению автора, общепризнанная теория не дает ответа. Тут видная эксплицитная оценка, автор подчеркивает важность и необходимость альтернативной гипотезы.

Его критика усиливается следующим утверждением:

Finally, the question of how ordered reentrant flutter can apparently change into fibrillation and vice versa has by no means been settled.

Назван еще один вопрос, который остается нерешенным, и это усилено эмфатическим by no means и вводным finally. Все предложение в комплексе несет отрицательную оценку.

Иногда автор открыто привлекает внимание читателя: It is important to note that the concept of circus movement reentry….

В следующей фразе говорится о той теории, которую автор как раз и продвигает. Она «была впоследствии отвергнута большинством авторов, поскольку считалась ошибочной».

This concept was subsequently rejected by most other authors because it was considered flawed on the grounds that…

Flawed (spoiled, deteriorated, corrupted, ruined, marred, damaged) - само по себе слово несет в себе отрицательную коннотацию.

was considered (предполагалось)- здесь ключевое слово. «Считалась» ошибочной – имплицируется, что на самом деле она таковой не была.

  И это становится очевидным из следующей фразы:

Nevertheless, the circus movement concept of AF is but one example of the truly inspiring deductive abilities that Lewis and his contemporaries demonstrated in their electrocardiographic analyses of cardiac arrhythmias.

Nevertheless – тем не менее (подразумевается - несмотря на то, что эта гипотеза была отвергнута как ошибочная) but one example – лишь один пример (подразумевается, что могут быть и другие).

the truly inspiring deductive abilities - автор показывает эксплицитную похвалу дедуктивным способностям Льюиса.

В предложении и имплицитно, и эксплицитно выражена положительная оценка.

Прямая оценка выражена и в следующем предложении:

Today, modern electrophysiologists have the great advantage of being able to access highly sophisticated tools to map cardiac electrical activity and directly observe the nature of the fibrillatory waves with unprecedented spatial and temporal resolution [32,33].

Оценка здесь выражается через следующие лексические средства: great advantage – (A condition or circumstance that puts one in a favorable or superior position )

highly - (very, extremely, strongly) sophisticated - (complex, intricate, tricky, modern, advanced)-оба слова содержат в себе эксплицитно положительную коннотацию.

Unprecedented - (unparalleled, extraordinary, unheard, unmatched)  - префиксальное прилагательное. Чаще префиксальные средства оценки обладают отрицательными коннотациями. Слово нейтральное и  может нести как положительную, так и отрицательную коннотацию. Автор говорит  об аппаратуре, которая позволяет достичь невиданного ранее разрешения. В данном предложении  присутствует положительная оценка аппаратуры.

Т. о., говоря о современных электрофизиологах, которые имеют большое преимущество в том, что имеют доступ к высокотехнологичному оборудованию, автор эксплицитно высказывает положительную субъективную оценку.

Далее автор обещает продемонстрировать доказательства правильности гипотезы Льюиса:

As we shall demonstrate below, it is remarkable that despite some qualitative modifications needed to match theory with experimental fact, the newer studies demonstrate that the original concept put forth by Lewis for the nature of AF remains basically correct, at least for certain forms of AF.

(it is remarkable-wonderful, great, admirable, marvelous, splendid, remarkable, extraordinary, outstanding, prominent) - видим явную, эксплицитно выраженную положительную оценку автора.

В следующем разделе встречаем эксплицитную положительную оценку автора:

Clearly, the anatomy of the atria, with its complex lattice of pectinate muscles, vein orifices and atrioventricular rings is an excellent substrate for sustained circus movement reentry, as originally postulated by Mines [12] and subsequently by Lewis [31].

В данном пункте автор делает аллюзивные отсылки к исследователям 1973и 1990-х годов. 

Clearly - автор открыто выражает свое мнение, что некий медицинский факт очевиден и не вызывает сомнений. Он  подкрепляет его положительными оценочными высказываниями типа: an excellent substrate-отличная основа. Cамо прилагательное excellent (extremely good, outstanding) имеет яркую положительную окраску.

Studies in animals at the macroscopic level suggest that the complicated three-dimensional structure of the atrium is an essential component that contributes to the complexity of propagation patterns identified by high resolution mapping during AF [25, 44, 45].

В данном предложении автор поясняет читателю, что сложная трехмерная структура предсердия является существенным компонентом. В словосочетании an essential (absolutely necessary, extremely important) component  - существенный компонент – автор привлекает внимание коллег к тому, что считает  чрезвычайно важным.

However, the information about how heterogeneous electrophysiology and heterogeneous anatomy interact to lead to AF initiation, maintenance or perpetuation is incomplete at best.

However, the information …. is incomplete at best - Слово incomplete-префиксальное прилагательное. В большинстве случаев префиксальные средства оценки передают отрицательную коннотацию. Слово несет в себе отрицательную коннотацию. Автор пишет про информацию, которая в лучшем случае является неполной. at best (в лучшем случае) предполагает, что может быть еще и в худшем случае, т. е. здесь налицо импликация и худшего варианта.

Следующие фразы стоит рассматривать комплексно:

Advances have occurred in the understanding of geometrical factors, such as wave front curvature [46] and sink-source relationships at areas of tissue expansion [47], and in the application of nonlinear dynamics theory to the spatial and temporal organization underlying complex cardiac arrhythmias [48,49], particularly during both atrial and ventricular ch advances may be relevant to the ultimate understanding of the mechanisms of initiation of AF by the interaction of the propagating wave fronts with anatomic or functional obstacles. -

Мы видим положительно коннотированнные существительные Advances (achievement, progress, promotion; достижение, прогресс). Автор рассказывает читателю о достижениях в понимании неких фактов, и в следующем предложении ссылается на них же:

Such advances may be relevant to the ultimate understanding - Такие достижения могут быть важны для окончательного понимания…

Здесь мы видим relevant (значимый, существенный) – констатация (оценка) важности. Также видим may - модальность, которая выражает неуверенность: «могут быт важны».  Автор показывает свою неуверенность в заявленной важности означенных достижений для окончательного понимания.

В следующем разделе автор подводит итоги изложенных выше исследований, и говорит о том, что они показали, подчеркивая  важную роль  (play an important role) структурных неоднородностей в мускульной структуре для ….

Thus, several investigations [24, 28, 42, 50] have focused on the role of the subendocardial anatomical structure of the atria in the mechanisms of cardiac arrhythmias. Рассмотрим следующие интересующие нас моменты: Altogether, the above studies support the importance of - В целом приведенные исследования подтверждают важность анатомического строения...

Автору важно показать, что его исследования подтвердили гипотезу.

Слово insufficient - (inadequate, insufficient, scarce, deficient, poor, low) обладает отрицательной коннотацией (как и большинство префиксальных прилагательных). Речь идет о том, что неких различий  недостаточно для того, чтобы объяснить механизмы устойчивости ФП.
