9. Состояния, при которых больные отказываются от еды, например невротическая анорексия.

10. Сердечная, почечная или печеночная недостаточность у отдельных больных, когда возникают специальные (особые) обстоятельства, обусловливающие необходимость парентераль­ного питания.

Принципиальные положения методических рекомендаций следующие.

1. Средняя калорическая потребность взрослого человека в норме составляет 24 ккал/(кг-сут) — 100 кДж/(кг-сут).

2. Объем парентерального питания увеличивают при перито­ните на 20—40% по сравнению с должным; при септических состояниях — на 5—8% на каждый градус температуры тела сверх 37 °С; при обширных ожогах II—III степени — на 50— 100%; при сложных переломах крупных трубчатых костей — на 10—30%.

3. Минимальная суточная потребность организма в азоте составляет 0,25 г/кг. Отношение небелковых калорий к азоту (ккал/г) должно составлять 200:1.

4. Минимальная суточная потребность в электролитах: натрий 2,4 ммоль и калий 2,0—2,2 ммоль на 100 вводи­мых кал/сут. Остальные электролиты вводят согласно нор­мам, приведенным в табл. 10.2. Минимальная потребность кальция составляет 400—600 мг и фосфора — 500—1000 мг на 2500 ккал/сут.

5. Умеренная передозировка водорастворимых витаминов не имеет существенного значения — интоксикация не возникает да­же при выраженной почечной недостаточности; передозировка жирорастворимых витаминов A, D и Е может вызвать инто­ксикацию.

6. Парентеральное питание не должно использоваться как метод повышения ЦВД и поддержания объема циркулирующей крови.

7. Все питательные ингредиенты вводят только через кате­тер, расположенный в одной из центральных вен (кава-катетер).

8. Все растворы, вводимые через катетер, должны быть по­догреты до температуры человеческого тела, и вливание долж­но осуществляться через системы одноразового пользования.

9. Каждый флакон с раствором аминокислот и взвесью жи­ров должен быть внимательно обследован визуально. Недопу­стимо наличие взвесей и хлопьев в растворах, а также выпаде­ние жировых скоплений на стекле флаконов.

10. 10% и 20% жировые эмульсии имеют одинаковую (нор­мальную) тоничность и могут в связи с этим иногда перели­ваться в периферическую вену.

11. Азотсодержащие препараты вводят медленно (не чаще 60 капель/мин) и обязательно совместно с источником энер­гии (глюкозой, жировой эмульсией, спиртом).

12. Жировые эмульсии переливают со скоростью 10 ка­пель/мин в течение первых 10 мин, 30 капель/мин в течение

последующих 10 мин, далее — не быстрее 100 капель/мин при условии хорошей переносимости жира. За 2 ч может быть пе­релито не более 50 г чистого жира. Более 100 г жира в сутки (500 мл 20% жировой эмульсии интралипид) вливать не сле­дует.

13. При вливании жировых эмульсий обязательно добавля­ют в каждый флакон 2500 ЕД гепарина и 15 ЕД инсулина.

14. Скорость введения глюкозы не должна превышать 0,5 г/(кг-ч). Инсулин целесообразно вводить подкожно из рас­чета 1 ЕД на 3—4—5 г сухой глюкозы.

15. В растворы в официнальной расфасовке можно добав­лять путем прокола пробки: в жировые эмульсии — гепарин, в растворы аминокислот — препараты витаминов, в глюкозу —все остальные ингредиенты инфузионной терапии.

16. Парентеральное питание осуществляют только в дневное время. Целесообразно начинать в 9 ч утра и заканчивать в 16 ч.
